During the COVID-19 pandemic, she began posting videos on TikTok, where her "Days of Girlhood" series documenting her gender transition propelled her to widespread recognition. She has also walked in New York Fashion Week for The Blonds, won a Streamy Award for Breakout Creator, received Attitude's Woman of the Year honor, and was named to the Forbes 30 Under 30 list.

Expanding into music and writing, Mulvaney released the singles "Blue Christmas" and "Days of Girlhood," the latter benefiting The Trevor Project, and premiered her solo musical The Least Problematic Woman in the World, which played the Edinburgh Festival Fringe before an Off-Broadway run at the Lucille Lortel Theatre in 2025. She also appeared in Apple TV+'s The Buccaneers, joined the cast of The Drowsy Chaperone at Carnegie Hall's Breaking the Binary Theatre Festival, published her memoir Paper Doll: Notes from a Late Bloomer in 2025, and made her Broadway debut as Anne Boleyn in Six at the Lena Horne Theatre in 2026.
